How trusts can increase reporting of Patient Safety Incidents

Dear Reader

Roughly 10 per cent of patients admitted to British hospitals suffer a patient safety incident. Most of these incidents are minor and transient, but a number prove severe or fatal.

However, 90 per cent of patients receive incident free care, often delivered to an extremely high standard.

A small group, backed by the executive at County Durham and Darlington Foundation Trust, led an initiative to focus on how to capture when "everything goes well" and create a culture of acknowledging and learning from excellence.

As a result of this, 60 per cent of those involved felt they were more likely to report Patient Safety Incidents. To find out how they achieved this result, download the full article for free.
